TRAVEL CLAIM REQUIREMENTS

GENERAL REQUIREMENTS (REQUIRED IN ALL TRAVEL CLAIMS)
▪ E-ticket – initial ticket purchased - round trip
▪ Passport (if travelling abroad)
▪ I.D. (if travelling locally)

Accidental Death, Dismemberment, and/or Disablement
▪ Death Certificate
▪ Notarized Incident Report
▪ Medical Certificate (in English or Filipino)
▪ Birth Certificate of Assured
▪ Birth Certificate/Marriage Contract of Beneficiary
▪ Certification of Disablement from Doctor (for disablement)
▪ Photo of amputated member (for dismemberment)

Emergency Medical Treatment
▪ Incident Report (for accident related injury)
▪ Medical Certificate (in English or Filipino)
▪ Original medical receipts (in English or Filipino)

For Emergency Trip Cancellation:
▪ Medical Certificate of assured or family member
▪ Duplicate copy of death certificate of the immediate family member (if family member died)
▪ Birth certificate/marriage contract of the assured (depending on the relationship of the assured
with the immediate family member)
▪ Certificate of cancellation of flight from airline
▪ Original receipts of pre-booked and non-refundable lodging and transportation.

For Emergency Trip Termination:
▪ E-ticket – new ticket purchased
▪ Boarding passes or stamp on passport
▪ Duplicate copy of death certificate of the immediate family member
▪ Birth certificate/marriage contract of the assured (depending on the relationship of the assured
with the immediate family member)

Loss of Travel Documents
▪ Any document satisfactory to the insurer issued by the carrier for lost travel tickets or the
embassy for lost passport
▪ Copy of the replacement passport and/or lost travel tickets
▪ if loss occurred in such circumstances, an irregularity report from the airline or a police report
▪ official receipts of the travel and communication expenses incurred in getting a replacement
passport or travel tickets

Loss of Baggage
▪ Property Irregularity Report from the airline if the loss occurred while the baggage was under
airline custody
▪ Police report if stolen (reported within 24 hours from the time of incident)
▪ Affidavit of Loss
▪ List of items with brand, model, type and corresponding amount

Strikes and Aircraft Hijacking
▪ Certification from airline stating the occurrence of strike or airline hijacking (includes number of
days)

Baggage Delay
▪ Property Irregularity Report issued by the airline
▪ Any document satisfactory to the insurer stating the exact date and time that the baggage was
retrieved
▪ O.R. of expenses incurred (on toiletries and clothing)

Flight Delay
▪ Flight Irregularity Report from the airline
▪ Receipts of expenses (food, lodging, transportation)

CLAIMS PROCEDURE

1. Email all claim documents relevant to your claim benefit including the General Requirements to travelsure@malayan.com or claims-travelsure@malayan.com or call + 63 2 628 – 8557.

General Requirements are:

  • Itinerary Ticket
  • Any valid Government ID (local travel)
  • Passport (international travel)

2. Once claim documents are completely submitted, our Claims Department will evaluate your claim for 7 – 10 working days.

3. If claim is approved, settlement may be in the form of check (to be picked up in our Head office, Binondo) or bank transfer (offered for foreign residents only). Otherwise, a letter of denial shall be sent to the claimant.
